At your initial visit to our office, one of our Registered Dental Hygienists will examine your teeth, assess your periodontal health, take necessary x-rays, and professionally scale and polish your teeth. One of our Doctors will review your x-rays, perform and oral cancer screen, examine your teeth, and make an assessment of your general dental health. The Doctor will address any concerns you have about your teeth and discuss any treatment he feels is necessary to maintain your dental health.
Please assist us by providing the following information at the time of your initial visit:
- A list of medications you are presently taking.
- Your dental insurance card, if applicable. This will save time and allow us to help you process any claims.
- And,if you have been seen in another office within the previous 12 months, please notify us so that we can help you to obtain copies of any x-rays taken at that office and save you the cost of retaking them.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the initial visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics for pre-medication).
